Medical Records Department!
The policy of the Medical Records Department is to be responsible for reviewing the records for completeness and compliance with established rules and regulations regarding the records. However, it is in the Data Office that errors or deficiencies are discovered and notification given to the responsible parties for corrections. The Medical Record Department is also responsible for storing the records in a secure manner, retrieving the records efficiently, and providing access to the records by those so authorized.
The medical record files are located at the 6th Floor of Hope Center Administration office area, with locked doors and restricted entry, to prohibit intrusion by unauthorized personnel and promote security.
The facility must maintain clinical records on each patient in accordance with accepted professional standards that are complete, accurately documented, readily accessible and systematically organized.
Medical records management is one of the important ‘administrative’ activities of the Hope Center that support the day-to-day running of the program. Without proper medical records management the center will soon find itself in administrative and clinical difficulties.
Adequate medical records management practices must be in place to ensure the following outcomes are achieved:
• Creation of medical records
• Capture of medical records
• Disposal of medical records
• Access to medical records
• Locality of medical records
• Reliability of medical records
• Planning
• Reporting
• Policies, Procedures and Practices
Therefore, the Professional Practice Standards for Long Term Care are used to evaluate individual areas of the health record, serve as a guideline for content development of health record policies and procedures, and establish confidentiality of the health record and are to be used as a tool in the contemplation of procedural changes for cost-effective and more efficient operations.
