Tracer and Home Care Program !
The Coptic Hope Center for Infectious Diseases is a comprehensive HIV/AIDS management
program that offers its services free of charge. Integrated into the Center’s program
is the Tracer and Home Care Program, which has been created to better address the challenges
which cause our clients to stop attending the clinic or to take their ARVS.
The Hope clients who fall into the target categories listed below will be
assigned to field workers and traced to their homes to assess the reason for their
failure to come to the clinic to follow up in following months with home visits,
home bases care services, counseling services and interventions to monitor progress over time.
Target populations
Primary Target Group
• Adults on ARVs
• Individual with special needs: Physically or mentally challenged
• Children on ARVs and basic care
• Pregnant and post-partum mothers and their babies
Secondary Target Group
• Client in need of home interventions for:
o Disclosure
o Voluntary Counseling and Testing in the home setting
o Request for home-follow up
Objectives, Activities and Services of the Program
The overall aim of the program is to support better clinic and treatment adherence for critical or special need clients by providing home tracing and support to assess and mitigate the issues which are affecting their overall health and wellbeing. This will be expressed by the following activities to be conducted as part of the program
• Client tracing and home follow-up
• Assessments of the client’s physical and psyco-social condition along with barriers to clinic attendance and treatment adherence
• Provision of home based counseling and education for client or household
• Caregiver monitoring, support and counseling. If necessary facilitate caregiver selection to reinforce stable long-term client care in the home
• Home based VCT for household members or partners of unknown status
• Monitoring treatment adherence for clients on ART
• Delivery of home based care, basic hygiene and fist aid services and instruction
• PMTCT support and counseling follow-up for mother and child up to one year
• Mitigation of psyco-social issues through counseling and/or referral to service centers
• Facilitate counselor assisted disclosure as requested by the client
• Nutritional counseling, education and monitoring
• Referral service for HIV or TB testing for client’s or household members
• Ambulatory services for immobile clients in need of medical attention
